This form needs to be submitted via email to accounting@vicsbingo.com or faxed to 1-703-991-8879, and it will only be once this documentation has been received and verified by the Accounting Department, that the credit/debit card will be re-enabled. When this procedure is completed successfully, your Vics-Bingo account will be credited with a $25 bonus upon your first deposit after the verification.
